Chronic periodontitis (CP) is an inflammatory disease of periodontium and is characterized by the loss of periodontal ligament and destruction of surrounding alveolar bone leading to tooth loss. Uncontrolled diabetes mellitus (DM) is a proven systemic risk factor for CP and can play a major role in its progression to advanced form within a short duration. There has been an increasing trend of DM type II patients seeking periodontal care in the recent past. Management of periodontitis in presence of hyperglycemic state is challenging, as there is varied response to surgical and non- surgical periodontal therapy among diabetics. Generally elective dental treatment is postponed until the patient's glycemic control is optimal. surgical regenerative procedure is not advised in patients with poorly or moderately controlled diabetes as it does not guarantee the successful surgical outcome. Use of Autologous platelet rich fibrin (PRF), first described by Choukroun et al is a new second generation of platelet concentrate used as a regenerative modality in periodontal surgery as it is packed with abundant amount of growth factors for promoting wound healing, bone regeneration, graft stabilization, wound sealing and hemostasis. Recently developedDUO QUATTRO centrifuge full system is a dedicated PRF generation unit which has the provision to prepare advanced forms of PRF with low- speed concept in centrifugation process. Advanced PRF(A-PRF) and injectable PRF(i-PRF) prepared from this unit promise predictable periodontal regeneration in patients with chronic periodontits without any underline systemic disease. However, there is a sparce evidence in the literature regarding the utility of newer forms of PRF in the regeneration of periodontium in diabetic patients. There is conflicting evidence with respect to platelet quantity and quality, thereby questioning its clinical application in diabetic population. There is limited literature regarding growth factor release from A-PRF and i-PRF of DM type II patients with CP and its activity on human gingival and periodontal fibroblast cells. Therefore, the present study aims to compare in vitro platelet quantification and growth factor analysis of autologous A-PRF and i-PRF from CP with controlled and uncontrolled diabetic patients.
